The City of Tucson will issue the appropriate activity number when you come into our office.
The information necessary for each application is listed on the individual forms. All of these
forms can be printed out and brought into our office.
All forms are in PDF format. Several of these forms can now be filled out online and then printed. They
cannot, however, be e-mailed to our office or saved to your computer.

The first is for the “Principal” and the second is for the “Surety”. Both signatures
are required to be notarized in both places.
