How do I request a reasonable accommodation?
If you have a disability you may request a reasonable accommodation by having your physician complete a Reasonable Accommodation Request form and returning it to us. Your doctor or other service provider will need to verify your need for the accommodation.

Central source of information about community resources, health and human services for the people and communities of Cochise, Graham, Greenlee, Pima, and Santa Cruz Counties. Information Specialists provide information to help people find and obtain the social services they need. Information Specialists help callers clarify their needs, provide referrals to the appropriate organizations and, if needed, will advocate on callers' behalf.
