Please email your completed Notice of Claim form to the City Clerk at CityClerk@tucsonaz.gov.

Deliver
Claim forms can be hand-delivered at the Tucson City Clerk’s Office at  255 W. Alameda, 9th Floor.

Be prepared to provide the exact location of hive and street address.

If you are an insurance company inquiring about a subrogation demand for damage to city property, please send an email to       RiskManagement@tucsonaz.gov.  Please include the following (if available):
  • Date of Loss
  • Type of damage
  • Loss Location
  • Name of insured
  • Police Report Number
  • Company’s Claim Number
